There's one supplied with the Exchange 2000 Resource Kit; I couldn't find an E2K3 version. There was also one supplied with a TechRepublic article by Rick Vanover but I don't know if you would still find it on their site.

My organization is about to start an Exchange migration and I was wondering if anyone knows where I can get a migration plan that I can use as a shell for planning this upgrade.
