A common reason behind account lockouts is users changing their password but being logged in to mutltiple workstations, or leaving a Terminal Server session (or RDP) which means that the open TS session has the old password cached and will lockout the user.
Another possibility is mapped drives or printers with the old password, but I think this is a lot less common.
