exuse my ignorance, but what is the "AD Mapper Tool" and where can I get it? I would say that if you poll 100 companies, you are going to get 100 different formalized documents. Given that you have nothing, and time is getting shorter by the day, decide on a format that you like, and produce.  You've provided the info - let those that come behind you 'formalize' it in the format that they want. (I suspect it won't be changed, and you will be setting the standard...)
