Ah, I see what you're saying. Right, so I think what it actually does is the equivalent of an MSI repair, where the patched components represent a "repair" of the app. So, yes, I think you are correct that only the patched components are updated. The other thing that happens is that the updated msi instruction file should end up being copied to and updated on the client as well (cached under c:\windows\installer) so that any subsequent repairs work against the patched version.

Right. My understanding of how the patch mechanism works in Windows Installer is that if it get the redeploy and it detects that it was patched, it should only execute the code to install the patch, not the whole app over again on the target machine.

I haven't seen it work that way for GP-deployed apps. You need to proactively do a "re-deploy" on the package to trigger that.

Why would he do that? The MSI is on the share for the GPO either directly or as the result of an administrative install. The correct procedure is to apply the patch to the distribution point with msiexec and I believe the /p switch and then Use the redeploy option by right clicking the app in the gpo it was deployed via.
