Mark, I had a similar experience back in 2001 at Force10 Networks. I would strongly recommend that you contact Microsoft Premier Support on this issue.

I have ran the clean up of lingering objects multiple times with no joy. I manually deleted the corrupt machine accounts via adsi edit and the ad then skipped onto the next set of failing attributes.
