There's quite a few, widely available, well supported, templating systems
available. HTML::Mason, HTML::Template, Template-Toolkit, HTML::Embperl,
Apache::ASP and probably a lot more.
For trivial forms like your example, use HTML::Template. Otherwise, I'd
recommend Templat-Toolkit.
