I have changed it as following

But still I am unable to GET this url into my example1b.pl script!

You might also want to double check your HTML. The link you have written
as:

href="c:\1xml\request_form.pl?id=1&lastmodified="12-10-03"&name=world

might not have the behavior you're expecting when a remote client 
activates the link.
