No need for a piece of test code...
perl -M<Module> -e0
will complain if the module can't be found (see 'perldoc perlrun').

Depend what information you want, and what you mean by a "nice report".
However ...
For version
perl -v or perl -e "print $]"
For @INC
perl -V or trying to load a missing module (see above)
To list modules installed by ppm
ppm query *
To list modules installed using the module INSTALL instructions (i.e.
perl Makefile.PL; make; make install)
perldoc perllocal
