I'm not sure if this is what you're looking for, but I've attached screenshots 
of the assessments we have for new or significantly revised policies. These are 
filled out by the policy owner and submitted at the time they forward their 
draft to my office for review (the first step in moving the document through 
the approvals).

In the screenshots, you will see every possible question. The assessments have 
logic built into them, so depending on how the questions are answered, some of 
them wouldn't show up for the person filling it out.
