For Indiana University, the policy cycle is three (3) years and is noted in our 
policy titled "Establishing University Policies" (go here: 
http://policies.iu.edu/policies/categories/administration-operations/policy-admin/establishing-university-policies.shtml).
  Specifically, in the Policy Statement section, scroll down to item 9; and, in 
the Procedures section, scroll to item 3, to see specific language notating 
this requirement.
