This happened to me one time, and I realized it was the size of the still, for some reason certain 'size' used in premiere pro 1.5 would come out empty and when you go back to the timeline and look at it, it would just appear green, when I resize it (make it smaller) it worked.

Here's a new one for me: I am editing a trip that my daughter's high school took to Disney World. I am using high energy songs and I have video and still photographs matching the beat of the music. Everything works well but for some reason, the last 7 stills do not show up when I compile the movie. They are in a string of about 17 stills, the first 10 show up, but these do not. The last one fades into the remaining video clip, and that clip shows up fine. These still are on the timeline and show up on the monitor when I scrub through them, but they do not compile.
