I don't have a suggestion for fixing the USB mic, but I can suggest a 
workaround if you're trying to add a voice-over: Record your 
narration in another program then import the audio file you recorded 
into Premiere. If necessary, you can use the very simple Microsoft 
program Sound Recorder, available under the Windows Start menu in the 
All Programs menu, System sub-menu, Entertainment sub-menu.

I find recording it externally easier because I can then tweak it in 
a program like Soundbooth for best audio quality. Once it's imported 
into Premiere, I slice it up into phrases (it's easy to see where 
phrases begin and end by looking at the waveform) and drag phrases 
into exactly the location I want. Slicing lets me get rid of 
breathing and other extraneous noise between phrases.

Narrating this way typically requires a script. If you want to react 
to the video while you speak off the cuff, then I hope you find a way 
to get your USB mic working with Premiere.
