I've done an upgrade from 4.1.3 to 4.2.0.0 and them 4.2.1.6. If I remember well, in the readme it's says that the 4.2.1.6 have to be apply on a 4.2.0.0 (at least). Take care that licence file are only include in the 4.2.0.0 distribution CD.
