Yes, there is. It can happen if you have a drive whose element ID mapping to rmt device name has gotten out of sync with each other. More common with new installations, but has happened to at least one other person on this mailing list when his FC switch broke and was replaced. It's pretty easy to distinguish from other cases because this will usually affect all or many tapes, rather than just a single tape. But I imagine the sheer majority of these incidents will normally be either a tape going bad or its label becoming unreadable. -Dan
