David, Upgrading from 4.2.1.9 to 4.2.3.0 there should be no need to issue the upgrade db command.
If you were upgrading from ADSM 3.X or TSM 3.7 to TSM 4.2.X. During the upgrade the database should be automatically upgraded but sometimes this does not happen. When you start the TSM server you'll get a message about "upgrade db." If you get the message, I forgot what the ANR# it was, then manually issue the upgrade db command.
