We have the same library as you, are the label prefixes different on the media types and have you set a cartridge assignment policy which means that the library won't take the media into the logical library without being told what the sequence of labels are???

The TSM server is at 5.5.4.0 on a Windows Server 2003 machine. The tape driver has update to the latest version 6212. I have recreated all the drives and paths.
