I've had this happen when the media was ejected using "move media" but checked back in after the volume went empty. There must have been some kind of disconnect between the library manager and client but it has happened to me multiple times in this specific scenario. An "audit library" from the library client may help, but to fix this for sure, checkout the tape first (checkout libv) so the q media does show "mountable not in lib". then do a move media command on the volume and that should delete it.
