This isn't so far off. In a large project you need, IMHO, one or two (or more depending on the number of people on the project) core developers who approve every patch. They can ask for changes to the patch (as Stas Bekman often does for mod_perl) or refactor it to meet coding standards (as I often do for Bricolage). Either way, the goal is to have a few trusted folks who ensure the quality of the code that goes into the repository.
