Hi Pete,

Thanks for providing me with extra detail around this issue.  It would
be difficult to diagnose this problem if it is not reproducible.  The
error might have been related to the account creation problem, but
it's hard to say with any certainty.  Let me know if you see this
happen again, and hopefully with more data points we can get a better
idea of what's going wrong.
