You may use the campaign_extension_setting report (https://developers.google.com/google-ads/api/fields/v13/campaign_extension_setting) and filter by campaign_extension_setting.extension_type (https://developers.google.com/google-ads/api/fields/v13/campaign_extension_setting#campaign_extension_setting.extension_type) is of value IMAGE. You may check on this report and if there are no records found, then that specific campaign does not have image extensions created for the said campaign.
