With regard to your below questions:
1. Can I assume that it was successfully uploaded but the conversion failed?

If the uploads via the API are successful and the same can be seen in the UI, 
then there is no reason that the uploaded conversions are failed.

2. Does this mean there might be issues with my data when uploading offline 
user data for Customer Match? How can I verify that I've successfully uploaded 
the Customer Match to the corresponding Advertisement?

Are you asking this question in the context of the same conversion upload 
operation as above, or is this question related to another operation where you 
are trying to upload the customer match data via the API?
