That's a lot of questions, so I'll try to answer as best as I can. If you 
have followup questions, you can ask them on this thread.

1) Yes. Try a CampaignService.get() to get all the campaigns. See 
http://code.google.com/p/google-api-adwords-java/source/browse/trunk/examples/v201109/GetAllCampaigns.java
 for 
an example.

2) Try retrieving the campaign targets and REMOVE ing the GeoTargets you 
don't need.
3) Try REMOVE ing the targets you don't need and then ADD the targets you 
need. SET doesn't work with v201109 Campaign criteria. v201101 is 
deprecated and will be discontinued in about 2 months. See 
http://googleadsdeveloper.blogspot.com/2012/01/55-days-until-adwords-api-deprecation.html
 for 
details.
4) This depends on a lot of factors like your internet speed, but at the 
Adwords API server, I guess it will take a few hundred milliseconds. You 
can check the responseTime response header to find out the time taken by an 
API call.
5) Yes, the response from AdWords API production and sandbox are identical 
in structure, it differs only in terms of the actual returned objects.
