Can you please try running the GetRefreshToken 
example<https://code.google.com/p/google-api-ads-java/source/browse/examples/adwords_axis/src/main/java/adwords/axis/auth/GetRefreshToken.java>
 via 
the command line with the 
adwords-axis-example.1-17.0.tar.gz<https://code.google.com/p/google-api-ads-java/downloads/detail?name=adwords-axis-examples-1.17.0.tar.gz&can=2&q=>
?

)

I also tested using the same bundle and it worked well. 
If it works from the command line, there should be an environmental issue 
with your web application setting.
