Thanks for the info. I did receive the email on Jun 19th and did follow the directions. I had filled out the form at https://services.google.com/fb/forms/newtoken2/ some time ago. I just submitted it again.
However, when I click the Submit button, it sits there loading for about 2 minutes and then just stops. There is no indication it actually submitted. This is what happened last time I submitted it as well. I am using Firefox on Windows 7. Is there a way to see if the form actually was submitted and received? Thanks!
