The most common cause for delay is that the token review team is still waiting for that form. In your particular case, did you fill out the form in the October 14th email with the subject "ACTION REQUIRED: Your AdWords API Developer Token (...)"? Once that form is in you should hear back from the token review team. If you already completed that step then please reply to that email so someone from the token review team can follow up with you.
