Joshua, Could I trouble you to send me the full request/response log by using "reply to author"? I'm not aware of the "applicationToken" header, for example. Please review Reporting Basics guide<https://developers.google.com/adwords/api/docs/guides/reporting>for the HTTP headers that can be used.
Also, please see if using a newly generated refresh token would resolve this?
