Could you confirm if you tried using the ad_group report and including some of the geo related segment fields? If not, please do so and let me know how it goes after. For this part of the issue, could you illustrate to us how this issue has been observed? Also, please provide the relevant request and response logs with request ID and request header generated on your end if you are using Google Ads API or report definition if Adwords API.
