We use mid on anything under half a mile.  Beyond that we follow Gino on this.

High gain everywhere.  Like Gino said, you never know when it's going to help 
in the RF environment, but it'll also save you on stocking additional parts.  
If all of your customers on that AP are the same radio and one of them goes 
down, you know which spare to bring.  Vehicles have pretty limited space.

2 miles is our general rule of thumb for installing mid-gain vs 
high-gain…..there are exceptions based on interference
