We have SMs that occasionally lose session, but not like that. I agree that it looks like the H-pol receiver is FUBAR.

What seems to happen most often (for us) is the SM re-starts session on the secondary color code and won't switch back to the primary. I have yet to find a combination for Color Code Rescan/Wait period that actually gets the SM go back to the primary after a reasonable amount of time. Currently using Rescan=30, Wait=2.

This is really weird. Anybody seen an SM get all outta whack like this? I'm pretty sure the radio is bad and we're going to replace it. It looks like fading, but it's not since it's been doing this day and night for a couple days. And it is losing session once in a while. The H pol power level will come back to normal every few minutes. What's really weird is the H power so low, yet the SNRs and fragments look backwards. And the uplink direction looks normal. There are other SMs on the AP that don't do this, so I have to assume this SM is just FUBAR.
