Surprisingly as recently as a few years ago when I was still doing DSL and FTTP, the DSLAMs often took PCMCIA cards to load initial configs and firmware. Ongoing provisioning always would go through vendor specific EMS platforms painfully typically out of band for telemetry, there wasn't a lot of standardization on the big iron equipment operationally. There's a few critical pieces that are standardized between DSLAM and CPE for PHY config handshake but that's not the config you're asking about.
But big vendors pre package an entire configured cabinet for them for a fee of course. On the CPE side, as the others mentioned TR-069, devices either come from vendors preconfigured to go to the right ACS cluster for the ISP or use DHCP options to pick up the address and start magical auto config. At one point I ran a cluster of about 15 million devices being managed for AT&T via the cloud. Spent a good number of years pioneering DSL plug and play install for CPE at 2Wire, but most ISPs had such different network capabilities due to years of legacy hybrids of vendors, and authentication techniques that only a certain amount could auto config and detect and go to even get to a TR-069 ACS. So many big ISPs needed special pre configs on the devices first to make the whole thing plug and play.
