The official P/N (with headphone) is ACATHS-01A. At one time at least it was made for Motorola by Best-Tronics. But it sounds like you want just the adapter. I tried making one myself once (actually repairing one) with a headphone jack from Radio Shack - that is not a good idea unless maybe your soldering skills are better than mine.
What I keep wanting to do instead is put a 6-pin modular jack on a little amplified speaker, and then just use regular telephone cords.
