Well, has anyone worked with someone like Neutral Tandem or the couple others like them? You're bypassing the legacy PSTN and using a third party tandem service. Obviously that only works for other people on Neutral Tandem, so kind of like peering vs. transit in the IP world, but would provide for some connectivity during an upstream telco outage. No idea if you can connect without being the CLEC. I believe quite a few of the cable and cellular companies are using such services.
