People selling telephone service in the US file an FCC Form 499-A. The instructions for form 499-A are linked below and they'll mention other taxation and regulatory fees and forms that must be dealt with such as USF, TRS, NANPA, LNPA, and ITSP fees.

I have no idea whether any or all of this applies to what you are doing, but if you are selling a telephone service within the US, I find it likely that Uncle Sam will want a piece of your pie.
