I've only introduced Trac/Agilo to a team of about 10 so far. Python is the latest 2.6 in Ubuntu. We do run under Apache with LDAP authentication and as a WSGI app, so that might not be the most common setup. I've so far been unable to predictably reproduce it, but it does happen fairly often. I'd be happy to test out any patches that you might want to send my way. (You already have my work email, so you can send it directly to me there.) I just tried reloading the creation form many times, but was unable to get it to fail of course.
