It was Cassini, IIS 6 fires the code everytime without a problem. Thanks to

If I may add my two cents, and say, Cassini and the Development web server
that runs in VS 2005, is a little buggy.  I was getting a number of problems
when accessing data from HTTPRequest object within a custom Httpmodule.  I

 The problems seems *not* to exist when the same code is execute under IIS6;
which leads me to believe that Cassini has a few problems.  So, I would try
and run the code under IIS 6.
