Is the driver side arm fully seated on the drive shaft? Sometimes, they get cocked, don't engage fully, and don't move all the way down to correct position. Be sure both those thin 13mm-drive nuts are equally engaged on their threads.

If all is otherwise correct, I'd try warping the flat part of the arm a little to create more pressure. YMWV, most certainly.
