hi graham.  have attached the super duper performatek installation 
instructions.  guaranteed to make your installation a breeze.  hah!
your experiences so far match mine.  i could not get the outside bushing 
off the shaft for anything.  bothered every one i know who all swore 
that the bushings just slide on and off - i was convinced they were all 
lying.  as you experienced, finally did get it off, but it was a struggle.

fwiw, easy to swing the axle to the side w the drive shaft disconnected 
- not as big a deal as it sounds.

as to fit, putting the bushings in right way around will help...
The bushings are identical but asymmetrical -- one side is  "deeper" 
than the other.  Deep side goes into the trunnion eye. use plenty of the 
supplied grease - inside and outside.  should fit fine.
the other real caveat, the factory torque spec for the nut is 80 / 108 
pound-feet*
*personally i do not understand how this can be... must be a misprint.  
when i tried it, the nut crushed the bushings until it homed on the end 
of the threads. maybe you are supposed to home the nut but it instantly 
went from some small torque reading to infinite - there was never any 
step when it read 80-108 #-ft.  no way the rubber part would do any 
better. in the end, i tightened the nut until the bushings looked 
compressed "enough" and the cotter pin had secure purchase in the nut 
castellations.  this seems to work just fine.
