Hi Colin, I'm pretty useless when it comes to diagnosing smog failures, but I can say I've never had my spider or my alfetta pass first time, and I only drive them slightly more than you do. I usually just drop it off with my mechanic and ask him to give the car a tune-up and at least pre-test it if he can't take it to the smog shop himself (I've heard differing stories as to whether the mechanic can actually take the car & test it).
Since timing and fuel delivery both factor into your emissions, any adjustments to (or degradation in) either of these systems could cause the problems you're having, and I would only be surprised at the failure if you'd replaced all the components in both... good luck with the re-test, and if you need a referral to a good mechanic, let me know.
