Been a while since i’ve used Total Recorder but I reckon that this App would 
most likely handle your job.
They way I see it all working is for you to set the source of Total Recorder to 
“What You Hear”.
Then set your microphone in such a way that you can hear that through your 
speakers though obviously in this instance you’d be best to use headphones to 
avoid any feedback.
So when Total Recorder is set to record “What you hear” will be recorded thus 
your Screen Reader, Microphone or anything else you care to add to the mix.
