A ) I have posted a similar bug ID#40637
B ) In OO 1.9.69 You can regenerate the error as follows
1- Start Draw
2- Right click and select slide design
3- Choose load
4- Select Presentation Backgrounds | Subtle Accent (or any other template)
5- Add something
6- Save and close
7- Open the file again
I have failed to generate it in OO1.9.71.1 because it crashes at step 4
