Thanks, cloph for persevering with this one. I think I have worked out the
problem, though I am not sure of the solution.

I added another "user" dictionary  extra.dic and set it to All languages.

Put an unknown word into it, and underline went, that is normal expected 
behaviour .

Removed the  "extra.dic", then reloaded a new document, and set up a new
"extra.dic" but set to English(Australia).  Now the behaviour was as before,
word is in the dictionary, but underline remains visible.

Of course, my "medical.dic" was in English(Australia). Paragraph WAS set to
English(Australia). I have now changed "medical.dic" to All, and working OK. 

If you want me to raise a new bug for this, I will, but it it is probaly so
minor as to not be necessary.
