Autocaption does not generate proper captions if the user selects the chapter
separator option.

Steps to reproduce:
-------------------
1. Turn on chapter numbering:  Tools - Outline Numbering - Level 1 - Number
1,2,3 - Separator After "."

2. Add a chapter heading to your document: From Style pane, double-click
"Heading 1", then in text area enter a chapter title, like "Chapter 1".

3. Turn on the table autocaption: Tools - Options - Writer - AutoCaption. Click
"OpenOffice.org Writer Table", verify that the Caption Separator is set to ": ",
and change the "Numbering captions by chapter" separator to "-".  Click OK.  

4. Use the create-table toolbar button to add a table to your document, below
your chapter heading.  

5. See that the table caption reads "Table 1-1-: "  The second hyphen should not
be there!

What should happen
-------------------
The number-by-chapter separator should be used after the chapter number ONLY,
not after both the chapter number and the table/figure number.  There is a bug
which is erroneously adding the number-by-chapter separator after both.

I think this probably slipped through because the default separator for both
caption fields is a period. However I would also argue that the default
number-by-chapter separator should be a hyphen, not a period, but that's not
really part of this bug.
