This issue appeared in the following document after changing from release 680m74
to release 680m77:

The Table of Contents is supposed to be set to use the text formatted as
"Heading 1" to create the Table of Contents automatically. However, this is not
saved properly.

To recreate, right-click on the gray in the Table of Contents and select "Edit
Index/Table". Then, under "Create from", click on the "..." to the right of
"Outline". Under "Paragraph Style", change "(none)" to "Heading 1". Click
"Okay", and then "Okay" again. Save the document, reopen it, and return to this
setting. "Paragraph Style" has reverted to "(none)".

This is a problem because the "Update Index/Table" function no longer works
after the "Paragraph Style" is reset to nothing. After setting the Paragraph
Style, right-click on the Table of Contents, select "Update Index/Table", and
notice that it creates the entries out the "Heading 1" headings. Then, close and
reopen the document, and select "Update Index/Table" again. It will erase all
the entries in the Table of Contents.
