This Issue replaces the closed Issue 59243

- Insert alphabetical index
- Entries: add 'chaper info' into structure line
- Index/table: activiate 'Combine identical entries'
but not 'Combine identical entries with..'
First problem: the 'chapter info' is reduced to only
one - the last - chapter in which the search string
appears.
Second problem: the 'chapter info' is activated
because each main-chapter starts with page numer 1.
But now it is impossible to know which numer-one-page
is the right, may also be more than one.
It should be possible either to consider the chapter-info as part of the entry
and leave one entry per chapter or to generate the page-nummer-list in a form
that the different chapters can be distinguished
entry chapter A 1,2,3
entry chapter D 2-5
entry chapter G 3,4,5
entry chapter A 1,2,3
chapter D 2-5
chapter G 3,4,5
