This issue was replicated.
a) step-by-step
1) open new spreadsheet
2) insert picture
a) select "insert" menu
b) select "picture"
c) select "from file"
d) chose any picture from the file system (I used the "intro.bmp" from
program folder in the OpenOffice directory)
(alternately you can drag-and-drop an image onto the spreadsheet)
3) select the "edit" menu, select "undo" (alternately use ctrl-z)
The toolbar that had appeared for the image remains until a cell is selected.
b) similar problem
Note that the converse is also true. If you add a picture and remove it via
the delete key or the cut option, the toolbar is restored to it's default
state. However, if you attempt to undo the removal the image is restored but
the toolbar does not update until you select the image.
1) open new spreadsheet

3) remove the image by tapping the "del" key on the keyboard (alternately

4) select the "edit" menu and select "undo" (alternately use ctrl-z)
The toolbar remains at the default state until selecting the image.
