Confirmed with 680m150 (8995), using Writer on a PC with XP SP2.
I have managed to replicate this bug using Writer. I believe it only occurs with
a specific sequence when saving the document. Since it is rather difficult to
reproduce, I will provide (later) the steps to re-create. I suspect certain
toolbar data is actually being written to the document file which shouldn't be.
