2005 -------
1. Start a new Writer document. 
2. Click Table > Insert > Table.. Insert a table with 4 rows, 2 columns. Your
options should have Heading, and Repeat Heading checked, for The first 1 rows.
The  remaining options don't matter.
3. Once the table is inserted, back in the Writer, you should see the Table
toolbar floating around (probably near the top of the document). Click on the
Insert Row button.
4. Type a couple letters in the first row to see that they are bolded and
italicized like you asked.
5. Type a couple letters in the second row. They are bolded and italicized too.

If you had pressed Insert Row a bunch of times, all of those rows would have
bold and italicized formatting set as well.
