2006 -------
If OOo Base crashed during designing a form the recovery of the form failed.
The is saved as part of the base file.
After the automatical recovery the form is an unknown writer file and has no
conection to the database which was open and is also recoveres.

If you didn't save the form once after creating it hasn't been recovered.

It's difficult to see that this is not the form which you use before the crash.
