sure ( been there done that ), and iirc it's in a difference module as well. IMO
the only way to catch this sorta thing is with instrumentation, bounchchecker (
is it still being used ? ) valgrind etc. tools are much better ( than us mere
humans ) at spotting those small hard to find errors like the above that can
cause the even harder to debug wild pointer misbehavior. In my case, I was lucky
it cored cleanly at the source of the problem.

It's unlikely that you will reproduce it, in my case I had a heavily stressed
system with alot of modules built with full debug. 
( I had forgotten about this issue, seems with the recent collapse-net problems
