1) Double entries: if you format chapeters with "Heading X" styles AND ALSO mark
them as TOC entries, they will be written 2 times in the TOC.
Only user the Style, not the Entry mark.
2) Yes, hyperlink which refer to targets containing an number and a dot "#. "
wont work. This is a known issue which still need to be fixed.
Advice: use the automatic chapter number instead of typing "1. ", "2. "
